Buffalo is a two-system town with a university in the middle. Kaleida Health and Catholic Health run large community hospitals, Roswell Park is the cancer center, and the University at Buffalo medical school anchors the Buffalo Niagara Medical Campus downtown. Systems like these employ a good share of the region's physicians, but independent practices remain, especially in the northern suburbs.

Local plans, local edits

Western New York has a payer mix unlike the rest of the state. Highmark Blue Cross Blue Shield of Western New York holds a large share, and Independent Health, headquartered in Buffalo, has a big regional membership. Both have their own portals and edits. A company used to downstate, where national carriers dominate, may not know either.

Medicaid in New York is delivered through managed care plans, each with its own authorization and timely filing habits. In Niagara Falls and on Buffalo's east side, practices bill a high volume of it.

The medical campus is only part of the story. Hamburg, Orchard Park, Amherst and Williamsville each have their own medical office parks, and the patients differ. Amherst and Williamsville lean commercial and Medicare Advantage, while the east side and Cheektowaga see more Medicaid. A practice in the southtowns near the Bills stadium may treat steel and manufacturing workers whose union-sponsored plans have their own claim addresses. Has the company set those up before? It should say so plainly.

No-fault auto and workers comp

New York has no-fault auto insurance, and a large share of physical therapy, chiropractic and pain management work in Buffalo flows through no-fault claims and workers compensation. These have their own forms, deadlines and denial paths, quite unlike regular health insurance. If you run a therapy or injury practice, raise this first. Some billers do no-fault work daily and do it well. Others will take your account and fumble it. Ask to speak with a client who uses the service.

Auto injury work also depends on timing. No-fault claims run against filing clocks, and a delay of weeks can turn a payable claim into a dispute. A biller should be able to tell you how it tracks those dates for each patient, not just that it does.

The Canadian border

Niagara Falls and North Tonawanda sit by the border, and some patients are Canadian residents or cross to work. Their coverage may not be accepted in the United States, which means payment at the time of service and plain statements afterward. Billing staff should know how to handle self-pay patients without chasing them in ways that drive them to another office. The same applies to Buffalo practices near the Peace Bridge.

Winter is a billing issue

Buffalo snow does more than close roads. A practice that loses two days of schedule to a lake-effect storm gets a spike in no-shows and rescheduled visits, and some authorizations expire before the new appointment. Strong billers track those dates so a visit lost to snow does not turn into a denied claim two months later. Ask each company how it watches authorization expiry and what it does when a storm shuts your office.

Where outsourcing fits

Smaller groups in the northtowns and southtowns are the obvious customers, along with solo behavioral health providers, who are plentiful in Western New York. Another group is physicians leaving large systems to go independent, who find that the system had been doing their billing in ways they never noticed. They start from scratch with credentialing, payer contracts and a clearinghouse.

Lackawanna, West Seneca and South Buffalo add older neighborhoods with long-standing family practices, many on Medicare and union retiree coverage. Those offices often keep paper charts and a veteran biller who knows every quirk. When that person leaves, the knowledge leaves too, and an outside company can write it down before it disappears.

Tonawanda and Cheektowaga practices, between the city and the Falls, tend to see the widest mix of plans, which makes a good test of any company's range.

Western New York also has an older population, and many practices are tuned to Medicare. Supplemental plans, referral rules and secondary billing deserve a direct question: how does the company post a secondary payment, and what does it do when primary and secondary disagree? In a market with this many seniors, the answer decides whether your receivables stay clean.

Questions for every company

Which software does it work in? Who works denials, and how do they report on them? What do you own if you leave? Where are the people, and what privacy training do they have?

Ask also about practices tied to the medical campus, such as groups that rent space near the hospitals and share referral patterns with them. They often have contracts and fee arrangements with the nearby systems that an outside biller has to respect. A company that treats the contract as a footnote will send claims out at the wrong rate.

And press on Medicaid managed care, where plans overlap in the same neighborhoods but not under the same rules. A good biller can say which plan needs a referral and which needs only an authorization without checking.

Compare scope, not the headline number. Pick a company you expect to still be with in five years. Enter your zip code and a few practice details, and you will get quotes to compare.
