Atrium, Novant and everybody else
Two health systems dominate Charlotte. Atrium Health and Novant Health run hospitals and physician offices across Mecklenburg County and well past it, and both employ a lot of doctors. Atrium is affiliated with Wake Forest School of Medicine, which brings teaching to the city. If you are an independent practice in Charlotte, you are surrounded by their clinics, and your referrals often depend on getting along with one of them.
That is a reason many small groups want to stay lean on overhead. Billing is where overhead sneaks in: an office manager, a biller, a part-time person who chases insurance, and a software license nobody has time to configure. Outsourcing replaces some or all of that with a fee.
A banking town with a big Blue plan
Charlotte is a finance hub, and banks and corporate offices fill the uptown towers. Many of their employees have employer-sponsored coverage, some self-insured and administered by a carrier. Blue Cross and Blue Shield of North Carolina is a major commercial carrier statewide, so expect your billing company to bill it often.
Medicaid in North Carolina has moved toward managed care, so practices with Medicaid patients now work with several plans instead of one state program. The transition was rough for some. A company with that change behind it is a better bet than one still learning.
Because so many Charlotte patients have steady employer coverage, deductibles are high and patient balances are a bigger share of your income than in a Medicaid-heavy city. Ask how the company handles statements and payment plans, and the phone calls that follow them.
Matthews, Huntersville, Concord and the rest of the ring
Medical offices follow the new housing. Matthews and Mint Hill on the southeast side have family medicine and pediatrics, with some orthopedics. Huntersville to the north has a growing number of specialists near the lake. Concord has a busy hospital campus and steady primary care. Indian Trail and Monroe further out are growing fast, with young families and urgent care centers.
Billing is remote, so none of this changes your fee. It does change your patient mix. Some practices in the southern suburbs see many patients who live across the state line in South Carolina, where Medicaid and some commercial plans work differently. If that is you, say so.
Inside the city, SouthPark and the area around Carolinas Medical Center and the Novant Presbyterian campus hold the densest clusters of specialists. Practices there share patients with the hospitals, so they bill professional claims while the hospital bills facility claims, and the two have to stay separate. University City on the northeast side has its own base of clinics, and Ballantyne at the south end has newer offices that serve affluent families.
How outsourced billing works here
The company submits claims, posts payments, works denials, sends patient statements and keeps credentialing current. You keep the front desk and the patient relationship. You pay a share of what is collected or a flat fee, usually by claim or by provider.
The best arrangements come with a monthly report on clean claim rates and how long money sits in AR. You are paying for results, so you should see them.
In Charlotte the staffing math is a big reason people outsource. Experienced billers are in demand in a metro full of large health systems, and a small practice that loses one can spend months finding another. A billing company is a way to stop depending on one person.
Many Charlotte practices start by outsourcing only part of the work, such as denials or credentialing, and keep posting in house. That is a fair way to test a company. Ask whether it will price a partial scope, and what it would cost to widen it later.
What to ask each company
Begin with how denials get handled. A weak answer is "we resubmit." A strong one names a person, a schedule and a report. Ask to see an example.
Then software: which billing and EHR systems the company uses every day, and what it takes to connect yours if it is not one of them.
On exit, ask who owns your data and what happens to open claims. Will it keep working them after you leave, and does it keep getting paid on them?
Last, find out where the billing staff are located and which of them have HIPAA training. Charlotte practices hear every pitch there is, so it helps to ask the same questions of each company and write down the answers.
Specialties
Charlotte has strong orthopedic and cardiology groups, and a steady supply of behavioral health clinics. Each has quirks, from global surgical periods to time-based codes. A general company that bills everything will usually be fine for primary care. For surgery or therapy, ask for a company that already bills your specialty and can say how it does it.
If you have two locations or several providers, ask whether the company can report by location and by provider. It sounds minor until you need it.
Charlotte keeps adding people, and some of them will land on your schedule. Pick a company that can keep up as you add providers, not only one that looks right at three.
Urgent care and primary care offices in Charlotte see heavy seasonal volume, with flu in winter and sports injuries in spring. Ask how the company handles a spike, since the work arrives long before the payment does.
